- Your phone number and a backup contact, your vet and the nearest 24-hour emergency clinic, a signed vet-care authorization, feeding amounts and times, medications exactly as labeled, the daily routine, and notes on fears, triggers and cues your dog knows.
- Can I use this for boarding or daycare?
- Yes. Boarding and daycare teams use the same information. Many also ask for proof of vaccines, so bring our free dog vaccination & shot record along with your vet's certificate.
- How should I list medications?
- Copy the name and instructions exactly as written on your veterinarian's label, and ask your sitter to initial each dose. Don't change amounts without your vet.
